Application Instructions and Forms
Charlotte Latin School provides online and printed options to make it easy for you to obtain forms and complete the admissions application process. You may choose to:
• Access, complete, and submit your application online
• Download printable forms and mail the completed forms to
Charlotte Latin
• Combine printable application forms and online teacher
recommendations
• Contact the CLS Admissions Office (select “Contacts /
Inquiries”) and request printed application forms via U.S. mail
(Packets and forms require Adobe Acrobat.)

You may print the application forms above or choose to complete the Online Admissions Application. By utilizing the online option, you will be able to monitor completed tasks online as they are verified by the CLS Admissions Office.
You may also submit the Scholarship Application and Applicant's Statement online.
Each application requires teacher recommendations. Teachers may complete and submit:

Please use the Transcript Release Form to request that your child’s current school sends copies of his or her school records to Charlotte Latin by February 15.
